Case of the month - July 2007 - discussion
Diagnosis - Diabetic Mastopathy
Features to note (click on the thumb nails to return to main images):
- Keloid-like hyaline changes in stromal collagen
- Lymphocytic infiltration of lobules - this was patchy
- Pervascular lymphocytic infiltration - this was patchy also
- Bizarre and enlarged stromal fibroblasts
Discussion:
- This constellation of histological features is almost pathognomonic of diabetic mastopathy
- These individual histological features may occasionally be seen in non-diabetics but very rarely together
- It typically presents as a painless non-tender mass lesion in insulin-dependent diabetics under the age of 30
- It has no malignant potential and simple excision may be offered if the patient wishes
